To Become a Patient

Step 1: 

We will need a current prescription from your provider. This prescription must be dated with a date no older than thirty (30) days.

Step 2: 

We will call you to gather needed information once we have a prescription.

You may also download and complete the intake forms found on our website on the page entitled "Forms" and bring them to your initial appointment.

Step 3: 

Once we have gathered your information, we will schedule you for an evaluation with one of our therapists at our office in Lee, Massachusetts.

Step 4: 

Please be sure to bring all of your insurance cards and a photo ID and a medication list with you to your first visit so we may make copies.

​Insurance:

Please advise us of your health insurance carrier(s) so we can be sure that we accept your insurance coverage.

Worker's Compensation/Motor Vehicle Accidents​: 

We need to have a current prescription on hand before we can do an intake and gather demographic information. You will also need to provide our office with Adjustor’s name and phone number, claim number, date of injury/accident, and insurance agency information. When we receive this information, we will reach out to the adjustor to get approval for an initial evaluation. Once we have that approval, we will contact you to set up your first appointment.

Preparing for your appointment:

Please wear comfortable clothing that will not restrict your movement. If you are using any physical aids (canes, walkers, etc.), please bring/use those for your evaluation appointment.

Cancellation Policy:

If you fail to keep your scheduled appointment or do not cancel within 24 hours prior to your appointment, we reserve the right to charge the patient $65.00. Gateway Physical Therapy and Wellness has the right to end your plan of care due to persistent absenteeism​.
